Hazardous Materials CUPA Program Forms

Effective January 1, 2013, all businesses that submit facility information such as hazardous materials business plans, underground storage tank, and hazardous waste generator forms and related documents, will be required to use the internet to submit this information to their local agency electronically through an electronic information management system known as the California Environmental Reporting System (CERS). Unified Program Consolidated Forms (UPCF) are no longer valid and will NOT be accepted by EHD.

Aboveground Petroleum Storage Tanks (APSA/SPCC)

Tier I facilities are those:

  • With no more than 10,000 gallons of oil, and
  • With no single container exceeding 5,000 gallons, and
  • Which has had no single discharge to navigable waters exceeding 1,000 gallons and no two discharges each exceeding 42 gallons within any 12-month period in the prior 3 years.

Tier II Facilities are those

  • With no more than 10,000 gallons of oil, and
  • Have a single container greater than 5,000 gallons, and
  • Which has had no single discharge to navigable waters exceeding 1,000 gallons and no two discharges each exceeding 42 gallons within any 12 month period in the prior 3 years.

California Accidental Release Prevention Plan (CALARP) 

  • Registration
    Required to be submitted for all facilities with a regulated substance at or above the threshold quantity
